Essential responsibilities include functioning as a key member of an inter-disciplinary team, providing parent coaching and support, scheduling and completing visits and follow-ups, assisting families in overcoming barriers with a range of concrete supports, teaching families key educational and developmental messages, documenting all activities in the patient’s record and care management system, communicating plans and updates with care team members, working in partnership with social workers, developing and maintaining strong community relationships, and engaging in ongoing training and learning. Qualifications preferred include a Bachelor’s Degree or equivalent relevant work experience, prior healthcare or community-based programmatic experience, bilingual skills in English and Spanish or Haitian Creole, and experience with care coordination/management. Required conditions include a pre-employment background check.
